However, if you were writing for an applicant for which you are already authorized or for a minor applicant and are their parent or legal guardian, you do not need to submit a Use of a Representative (IMM 5476) form or a Release of Information to an Individual (IMM 5475) form or a Power of Attorney. Instead, when filling out the IRCC Web form, please make sure to:
Enter their information under the Tell us about yourself section, and
Include the following information in the Tell us about your situation box of the Web form:
your surname and complete given name (including middle name, if applicable),
your date of birth (YYYY-MM-DD),
your country of birth, and
your relationship to the applicant,
Describe your situation, including what you need help with, and
Add the keyword UKRAINE2022 at the end of your description.
