This email is in reference to your status as a temporary resident in Canada. Upon arrival, and having received facilitation under the Canada-Ukraine Authorization for Emergency Travel (CUAET), you received a condition on your temporary resident status that was printed on your visitor record or work permit, instructing you to undergo a Medical Diagnostic Test within 90 days. Our records indicate that 90 days have now passed and we have not yet received your Medical Diagnostic Test results as required.
· If you have already completed your Medical Diagnostic Test, no further action is required at this time. Your panel physician will submit the results to us for processing.
· If you have already booked your Medical Diagnostic Test, please keep your scheduled appointment and disregard this letter. Once completed, your panel physician will submit the results to us for processing.
· If you have not yet scheduled your Medical Diagnostic Test, please contact a panel physician as soon as possible in order to book your appointment. Instructions for the Medical Diagnostic Test are included within this letter.
A list of all panel physicians can be found here: http://www.cic.gc.ca/pp-md/pp-list.aspx. The panel physician will arrange for your Medical Diagnostic Test, and will notify IRCC when you have completed the requirement.
Non-compliance with this condition on your visitor record or work permit within the next 60 days could result in you being contacted by the responsible authorities for additional follow-up and, where applicable, appropriate enforcement action.
