You should apply at your nearest Czech embassy or consulate.
Special considerations
• An official application form is available at any local or municipal office
• Original and valid personal identity document with a photograph (such as a national ID card, a passport or a foreigner’s residence permit) will be needed to process your request.
• If a representative is applying on your behalf, he/she must have notarized power of attorney (including a sworn translation in Czech if not already in that language)
• You must have your certificate translated into English or French.

Unless you apply directly at the Criminal Register office in Prague, the results will be returned to you by mail.
